Coordinate and manage various programs and services for seniors and community members Provide leadership to a team of home support workers including supervisory work related to staff and daily operations Monitor targets and compile quarterly statistic reports Conduct need and psychosocial assessments and therapeutic intervention/counselling to clients to assist them to achieve optimum social functioning and psychosocial wellbeing Develop, initiate and coordinate individualized support plans for clients and provide ongoing resource management and case management as appropriate Provide counselling services to individual and caregivers Advocate for clients and their families in matters affecting senior’s health and wellbeing Identify and support access to community, volunteer and professional services and programs for clients Perform cross-functional and other responsibilities, as assigned and/or requested
